# Speaking up for safety: the moment someone has to say it

A risk is noticed. Someone senior is busy. Practise the words, and the response, before anyone depends on them.

## A concern with nowhere to go

A nurse sees a patient getting worse. A counter worker watches a colleague treat a customer badly. A new starter spots a shortcut on site. Each knows something is wrong. The hard part is saying it to someone who outranks them, and being heard.

Why it goes wrong

## The words arrive too late, or too softly

People know the policy. What fails is the moment: a senior colleague who sighs, a busy shift, a concern phrased as a question that is easy to wave away.

### Each side of the conversation

A concern protects someone when the receiver acknowledges it, asks and acts. [AHRQ’s TeamSTEPPS curriculum](https://www.ahrq.gov/teamstepps-program/curriculum/mutual/teach/two-day.html) teaches assertion and escalation, and gives the person being challenged a duty to acknowledge it. So practice gives each role a turn.

### Ready-made VR titles

### Custom VR, set in your own rooms

Mater Education rebuilt its Speaking with Good Judgement program in VR, in recreations of its own hospital rooms. A [custom XR build](https://peopletechrevolution.com/xr) starts from your incident themes and your escalation pathway, and a [fast build](https://peopletechrevolution.com/fast-builds) puts a first version in front of staff quickly.

### AI roleplay in a browser

With [AI roleplay practice](https://peopletechrevolution.com/ai/ai-roleplay-practice), a nurse or supervisor raises the concern with an AI colleague on a laptop or phone, gets feedback against set criteria, and tries again in private.

### A facilitated session and debrief

A facilitator briefs the group, each person takes a turn, and the debrief finds the point where the concern could have been missed. That conversation is where practice becomes a habit.

In Mater’s own [program evaluation](https://peopletechrevolution.com/uploads/docs/people-tech-revolution-mater-immersive-learning-pilot-case-study.pdf) of a pilot with more than 60 staff, 90% of participants reported better concentration, 77% found the scenarios more engaging than face-to-face delivery, and over 96% wanted more training in VR. Staff felt a customer refused because of his sexuality, then practised speaking up as the colleague who saw it.
