# Does VR training work? Controlled studies measure real gains

Staff who rehearse in VR show measurable change against comparison groups, in hospitals and in teacher education.

## A budget meeting, and one question

An education lead asks to fund VR training, and finance asks one thing: will staff change? Controlled studies are built to answer it.

The controlled trial

## Nurse leaders, measured against a control group

A VR series for Mayo Clinic nurse and social worker leaders was tested the way a finance committee would want: trained leaders against a control group.

### Who took part

The [Social Science and Medicine](https://doi.org/10.1016/j.socscimed.2024.117648) trial started from 1,149 eligible nursing leaders. 841 completed the baseline survey and were assigned to the intervention or the control group.

The series was built with Equal Reality, and the [Mayo Clinic nurse leaders study](https://peopletechrevolution.com/work/research-evidence/vr-nursing-leaders-mayo-clinic) sets out its design.

### What changed

For empathic feeling and expression, the intervention group’s scores rose by an average of 0.11 points while the control group’s fell by 0.02, a difference significant at p = 0.002.

For empathic perspective-taking, the intervention group rose by 0.20 points and the control group by 0.02, at p = .01. Both measures moved further in the trained group.

## Pooled studies point the same way

A meta-analysis combines many studies, so it shows an effect that no single small cohort can.

### Teacher education

A [systematic review and meta-analysis](https://doi.org/10.3389/frvir.2025.1620905) of virtual reality in teacher education reports a positive moderate overall effect, Hedges’ g of 0.524.

For a buyer, a positive pooled effect is the signal to fund a pilot and measure it well.

## Regulators write practice into the rules

In several places the method itself is part of compliance, so rehearsal is what the rule asks for.

### United States

[California’s health care workplace violence standard](https://www.dir.ca.gov/title8/3342.html) gives staff an opportunity to practise with the colleagues they work with, including a meeting to debrief the practice session. [Nevada law](https://www.leg.state.nv.us/nrs/NRS-618.html) asks medical facilities for the same practice and debrief in de-escalation training.

Hospitals work to the [NSQHS Standards](https://www.safetyandquality.gov.au/standards/nsqhs-standards/clinical-governance-standard), which call for a [mandatory training program](https://peopletechrevolution.com/learn/standards/nsqhs-action-1-20) and [de-escalation strategies](https://peopletechrevolution.com/learn/standards/nsqhs-action-5-34).

Proving it at home

## Design a pilot that shows change

A first cohort proves the program runs. A comparison group proves people changed. Plan both before the first session.

### Answer the operational questions first

Does it run on your sites, will staff wear the headset, does the scenario match the work? A small first cohort answers those well.

### Then measure change

Compare trained staff with a control group, on a measure chosen before the pilot starts. That is the design the Mayo Clinic trial used.

### Keep the record

Controlled studies report measurable gains. In the Mayo Clinic trial, empathy scores rose further in the trained group than in the control group, and a meta-analysis of VR in teacher education reports a positive moderate effect.
